Yellow Fever Vaccination. Copy of International Certificate of Vaccination for Yellow Fever.

Doctor's note. Letter from physician confirming that the applicant is free of all communicable diseases.

Bank Statement. Copy of a recent bank statement showing proof of sufficient funds.
g
Personal Letter. A personal letter from the applicant addressed to the Embassy of Liberia, explaining the purpose of the trip, dates of travel, and cities to be visited.
